The Indian Health Service, Crownpoint Healthcare Facility , Service Unit is issuing this sources-sought notice (SSN) to identify capable Indian-owned, economic enterprises – large (IEE) or small (ISBEE) – capable of providing a turnkey commercial fitness equipment solution for the Crownpoint Healthcare Facility, Wellness Center. The anticipated requirement includes furnishing new, commercial grade cardiovascular equipment, strength-training equipment, functional training equipment, racks and benches, free weights, fitness accessories, and related equipment. The contract will be responsible for all necessary freight, inside delivery, assembly, installation, placement, set-up and testing to provide a safe and fully operational fitness facility. The contractor will also be responsible for coordinating equipment placement with the Government, performing required equipment anchoring and setup, verifying operational readiness, removing packaging and installation debris, and providing applicable user manuals, warranty and maintenance documentation, and basic equipment operation/maintenance orientation to designated Government personnel. Equipment must meet applicable commercial quality and safety standards and include appropriate manufacturer warranties. The Government may also require preventive maintenance services for the installed equipment. Other capable firms are also invited to respond to this SSN. The Contracting Officer will consider responses for Buy Indian Act (25 U.S.C. 47) opportunities first, then for set aside programs under the Small Business Act (41 U.S.C. 3104). Your submission must include the following information, at a minimum: Company Profile that includes: UEI, Name and Address, Primary POC information, business size and type of ownership, socio-economic status/participation such as 8(a), WOSB/EDWOSB, SDVOSB and HUBZone, identification of Native American ownership, SAM.gov representations and certifications, and other related documentation. A brief statement of the company’s capability that relates to the described government requirement. Additional demonstration of capability may include current and past performance history for the same or similar requirement, established contracts such as GSA or VA Schedule, etc. Indian-owned Small Business Economic Enterprises (ISBEEs) and Indian-owned Economic Enterprises (IEEs) must also submit a completed IHS IEE Representation Form (attached) in accordance with HHSAR 326.606-1(b). Relevant Experience/Past Performance. Provide up to three examples of contracts or projects completed within the past five years involving the furnishing and installation of commercial fitness/exercise equipment of similar scope, size, and complexity. Include the customer/agency, contract or order number if applicable, approximate dollar value, period of performance, brief description of work, and whether the respondent served as the prime contractor or subcontractor. Equipment/Supply Capability. Identify the commercial fitness equipment manufacturers or product lines the company is authorized or otherwise able to provide. Describe the firm's ability to furnish commercial-grade cardiovascular, strength-training, functional-training, free-weight, rack/bench, and fitness accessory equipment meeting the Government's requirements. The vendor package reflects a broad mix of these equipment categories. Delivery and Installation Capability. Describe the firm's capability to provide or manage freight, inside delivery, unloading, assembly, placement, installation, equipment anchoring where required, testing, startup, and removal of packaging/debris.
