This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation 36C25926Q0676 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through FY26.07B 04/28/2026, FAC 2026-01 03/13/2026. No telephone requests will be accepted. Only written requests received directly from the offeror are acceptable. The associated North American Industry Classification System (NAICS) code for this procurement is 339112 -surgical and Medical Instrument manufacturing, with a small business size standard of 1,000 employees. This solicitation is 100% set-aside for SMALL BUSINESSES (SBs). CONTRACT TYPE: Firm-Fixed Price. STATEMENT OF WORK FOR THE OKLAHOMA CITY VA HEALTH CARE SYSTEM Otoscopes/Opthalmoscopes Background The Oklahoma City VA Health Care System (OKC VAHCS) requires 13 Otoscope/Ophthalmoscope for Primary Care Service Women s Health Clinic. Objectives/Scope To ensure the safety and comfort of patients due to new facility. The exam will provide new equipment for the OKC VAHCS. The vendor will provide the equipment and required accessories for use at the Oklahoma City VA Health Care System. The vendor will inspect all equipment prior to delivery and installation; any concealed damaged components will be replaced at no cost to the VA. Purchase of American Made supplies remains a priority for the VA. Selection Criteria Must have Coaxial LED ophthalmoscope Must have LED otoscope Must include ear specula dispenser for disposable ear tips which attaches and hangs on wall Otoscope and ophthalmoscope must be wall mounted with holders for both Must have automatic on/off sensors Deliverables or Deliver Schedule The contractor shall deliver the units standard delivery with F.O.B. destination with 180 days of the date of the award. The equipment shall be delivered to the following address: Oklahoma City VA Health Care System - Warehouse 50 NE 42nd Street Oklahoma City, OK 73105 Documentation necessary includes a statement acknowledgement/invoice and receipt of delivery. Place of Performance The place of performance will be the Oklahoma City VA Health Care System Period of Performance The warranty shall be 1 year from installation.
