Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-3. This solicitation is set-aside for Service-Disabled Veteran-Owned Small Businesses. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 Surgical and Medical Instrument Manufacturing with a small business size standard of 1,000 Employees. The FSC/PSC is 6515 Surgical and Medical Instruments, Equipment, and Supplies. The Great Lakes Acquisition Center, on behalf of the Captain James Lovell Federal Health (FHCC) North Chicago, IL 60064 is seeking to purchase a one-time buy for DoVera DE12 Surgical Exam Lights. All interested companies shall provide quotations for the following items. Please note additional submission instruction found beginning on page 5 of this combined synopsis / solicitation. Inspection & Certification Any proposed or equal must have the following salient characteristics: Surgical Light Arms should have ample arm reach & pitch full 360° arm rotation and roll rotation capability of the light head and must provide high-intensity, shadow-free illumination with a CRI = 92 color temperature between 4,000 5,000K with a neutral clean white light that is comfortable for staff excellent for exams (4000K-4500K), operating at a low temperature, to reduce eye glare and strain for staff. The light must be mountable in VA dental operatories, feature sterilizable controls, and comply with all relevant VA infection control and electrical safety standards. LED lifespan should exceed 60,000 hours, with a minimum manufacturer warranty of 2 years and local service support. Delivery, FOB Destination, shall be provided by October 30, 2026.
