Nature of Requirements: The following is a summary of the statement of work for this requirement. The Correctional Service of Canada (CSC) Health Services requires the services of a Primary Care Physician (General Practitioner) for Warkworth Institution in the Ontario Region. The Contractor (General Practitioner) must provide Primary Care Physician services to inmates and collaborate with the Institution's inter-disciplinary Health Services Team that includes, but is not limited to, Nurses, Pharmacists, Nurse Practitioners, Social Workers, Dietitians, Dentists, Psychologists, and other medical and allied healthcare professionals. Objectives: Provide and actively participate in the coordination of essential health care services to patients at the Warkworth Institution. Deliverables: The Contractor (PCP) must provide integrated, trauma informed, culturally appropriate Primary Care health services to patients, as requested by the Project Authority, consistent with the Performance standards outlined in this statement of work.
